What you get…

Basically you will receive 2 things – a written assessment of your face and the enhanced versions of your pictures showing how you are likely to look after FFS. I will do my best to send these to you in about 4 weeks.

Using our volunteer Sam again, here are her 3 original pictures for you to refer to as you read her assessment.

The assessment:

Hairline:

Sam’s hairline is high and has moderate recession at the temples giving the classic male “M” shape. This can be brought forward by a scalp advance and/or hair transplants.  An advance of about 2 cm’s in the middle and about 3 at the temples should be enough.

Forehead:

Her brow-bossing is not very heavy and she does not have a strong “step” between the bossing and the rest of the forehead so overall it looks relatively smooth and moderately feminine. However, when viewed in profile you can see that her forehead does stand quite well forward of her eyes and needs to be set back a few mm’s to give a more feminine vertical profile. You can also see the protrusion of the orbital rims in the ¾ profile which again are not too bad but still need to be shaved down to bring them into normal female ranges. Forehead recontouring may also help eliminate the masculine frown lines.

Eyebrows:

Her eyebrows are already fairly high for a genetic male. However, there is still room for improvement and I would recommend a subtle lift to further open and feminise her expression (a little plucking will also help thin them out).

Cheeks:

Her cheekbones are a little flat at the front but they will stand out more when her forehead and nose have been set back a little. She could consider cheek implants to bring them forward a bit more and make them fuller but this is probably a very marginal consideration.

Nose:

Her nose is nicely formed – it is relatively small and narrow and already has a slightly concave profile. However, the tip of the nose comes to quite a long masculine point and she would benefit from having it made a little blunter. She could also have the tip turned up a little more but this would need to be done cautiously otherwise it could further emphasise the long top lip. Bear in mind that if she has her forehead recontoured it is very likely that she will need a full rhinoplasty anyway to set the nose back a little so that it blends naturally in to her new forehead. This does not mean that the overall character of the nose needs to change very much.

Top lip:

Her top lip is long and represents a significant opportunity for feminisation so a lip-lift should be a priority. She would also benefit from rolling the lip out a little to make it fuller and the surgeon may be able to make it a little more symmetrical at the same time as it drops a little on her left side. If you look at this area in profile you will see that the skin between her lip and nose is vertical. This is a masculine trait but the area can be tilted backwards a little during the lip lift.

Lips:

Her bottom lip is quite thin and will look even more so if the top lip is rolled out a little. Lip implants will make it fuller and more feminine and bring it in to balance with the top one.

Chin:

Her chin is her other big opportunity for feminisation. It is a little tall and it also has a wide flat base with 2 corners and a strong cleft in the middle – all classic masculine attributes. If you look at the profile shot you will see that it also recedes a little. The chin needs recontouring to reduce the height and bring it to a single feminine point. If she has a sliding genioplasty to accomplish this then the chin can be moved forward a little at the same time otherwise the recession would need to be fixed with an implant.

Jaw:

Her jaw and therefore the lower third of her face are quite narrow from the front which is an asset. She could take it just a little further with work on the jaw angles but this may be optional. From the profile and ¾ profiles it can be seen that the angle of her jaw is not particularly tall or square but again there is room for a little improvement.

Adam’s apple:

Her Adam’s apple seems to be well within normal female ranges so she will not need a trach shave.

Overall her biggest problems are her top lip and chin which together make the lower third of her face long and masculine so these are her priorities for feminisation.

Here are the pictures again, this time with enhanced versions alongside which have been adjusted according to her assessment:

As you can see, in Sam’s case the changes are subtle but reasonably effective. It is important to understand that FFS cannot always magically transform a face from unmistakeably male to unmistakeably female but any masculine face can be feminised to some degree and sometimes the transformation can be truly extraordinary.

It is also important to remember that in these pictures, Sam is in “male mode” with no make-up and a very short, masculine hairstyle so here is another altered picture of her with make-up and longer hair so that you can see how the changes might look in everyday life. Please note that Sam wore make-up and a wig for this picture and although I can add subtle make-up digitally I cannot usually add hair – I can only adjust and reshape the hair in a picture to a limited degree.

A picture like this would not normally be included - it is only here to illustrate the importance of your general presentation. If you want this kind of picture it is best to send an extra shot in which you already wearing make-up and the desired hairstyle etc.
